If you're planning to travel outside the Philippines and you're thinking about applying for a passport. Please check the following requirements:
1. Confirmed Online Appointment (click here)
2. Personal Appearance
3. Accomplished Application Form (click here for Blank Application Form)
4. Original copy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper
If you are married (females, using their spouse's last name), you must also submit the original copy of PSA (Philippine Statistics Authority) Authenticated Marriage Contract on Security Paper (SECPA) or Report of Marriage
5. Any of the following Valid IDs with one (1) Photocopy (click here for the List of Acceptable IDs for Passport Processing)
